Assessment and Referrals

BDNH Assessment Officers provide comprehensive assessment for every new client referred to our organisation to determine the most appropriate care plans. This includes providing support and education to clients, family and carers in relation to care needs. Our staff are skilled in assessment and care coordination (not case management which we are not funded for). We will work with your case manager, where you have one.

BDNH nurses can help you by:

• Assessing your health needs and helping you develop a care plan;
• Arranging specialist assessment if required;
• Helping you find and link with other community services if required; and
• Working in partnership with other health service providers to ensure optimal care is coordinated and monitored.

Which staff can help?

Our staff are skilled in assessment and care coordination (not case management which we are not funded for). If you have health needs that require specialist nursing assessment, our nurses can arrange that through one of our specialist nurses. Alternatively, we will refer you to another health care provider as appropriate.

Governor General Supports BDNH!

We were honoured to have the Governor-General, Her Excellency Ms Quentin Bryce attend our Commemorative Reception - the official close of our Centenary Year.

Ms Bryce commended the work of our organisation and took the time to meet with clients. A lovely afternoon was had by all!
